TERMS & CONDITIONS
WAIVER AND RELEASE OF LIABILITY CLAUSE:
1. ASSUMPTION OF RISK:
I understand and acknowledge that participation in dance classes, rehearsals, performances, and related activities involves physical exertion and carries inherent risks of physical injury. These risks include, but are not limited to, muscle strains, sprains, falls, broken bones, and other bodily harm. I voluntarily assume full responsibility for all such risks.
2. RELEASE OF LIABILITY:
I release [SUPERFLY DANCE STUDIO], including its owners, staff, and volunteers, from any liability, claims, or demands for personal injury, property damage, or death arising from participation, regardless of negligence.
3. MEDICAL TREATMENT AND EXPENSES:
I confirm I am in good physical condition for participation. I accept full financial responsibility for any medical treatment and understand that [SUPERFLY DANCE STUDIO] does NOT provide insurance.
4. INDEMNIFICATION:
I agree to indemnify and hold harmless the studio and its staff from any claims or legal actions, including attorney fees.
5. ACKNOWLEDGEMENT
I confirm I have read and understood this agreement and am voluntarily waiving legal rights, including the right to sue.
REFUND AND CANCELLATION POLICY:
Registration fees, Bookings, Memberships, Costumes, Recital/Performance fees, and/or Other studio-related payments are NON-REFUNDABLE. By enrolling in our studio, families agree to pay IN FULL at the time of purchase.
Refunds will NOT be provided for missed classes, schedule conflicts, withdrawal from a class or membership, vacations/holidays, sicknesses, or other changes in personal circumstances.
The studio MAY [at its sole discretion,] consider REQUESTS for refunds, credits, or account adjustments in extenuating circumstances...including [but not limited to] serious illness or injury, family emergency, extreme inclement weather, military deployment, and/or other unforeseen hardships. Documentation may be requested when appropriate and applicable.
Any approved refund, credit, and/or account adjustment will be determined on a case-by case basis by studio management and is NOT guaranteed.
WRITTEN NOTICE OF WITHDRAWAL FROM A MONTHLY MEMBERSHIP must be at least 30 DAYS in advance. Membership Fees already paid are NON-REFUNDABLE and families remain responsible for any charges incurred before the withdrawal date. Families are encouraged to send an email at superflydancestudio@gmail.com to request the withdrawal.
